Field Trip Information

Field Trip Request Form

The field trip request form needs to be submitted for all field trip requests. This will ensure that the trip has been properly run through the channels necessary to keep our district on the same page.  Overnight and out-of-state trips will additionally need school board approval.  Please plan accordingly to get approval in time for your request. You must print it off (or make a copy) and submit a copy by hand.

Effective 08/04/2025

Transportation Request Form

The transportation request form begins the process of arranging transportation for your approved trip.  Please be specific and clear as to the details of your trip to ensure that the proper vehicle (and driver) is reserved.  We expect transportation requests to be submitted at least two weeks before the date of travel.  This submission will also serve as a 'trip ticket' for the bus driver as he or she gets your group to the correct destination at the right time with all details needed for a successful transport.  Please fill out this form electronically through the Google Form process, and your request will be sent to those who need it.
